What we collect

When you submit a form on MN Therapy Finder — requesting help finding care, requesting info from a provider, listing a practice, or claiming a listing — we collect the information you enter: name, contact details, city or ZIP, insurance, the service you need, urgency, and any message you write. We also collect standard technical logs (such as IP address) to keep the site secure and prevent abuse.

How we use it

Family requests exist for one purpose: connecting you with matching providers. We share your request only with the provider(s) you choose or that match your stated city, insurance and service. We do not sell personal information, and we do not publish family information publicly.

Provider submissions and claims are used to create, verify and improve provider listings, which are public by design.

About health information

Please share only what's needed to route your request — you never need to include diagnoses or medical records in a form. MN Therapy Finder is a directory, not a medical provider, and information you send us is not a medical record.

Provider listings

Listing information (practice name, address, phone, website, services) comes from public sources and from providers themselves. Providers can claim their listing, request corrections, or ask for removal at any time by contacting providers@mntherapyfinder.com.

Cookies & analytics

We keep tracking minimal. If we use analytics, it is aggregate and privacy-respecting; we don't run advertising trackers on family-facing forms.

Your choices

You can ask us to delete a request you submitted, correct provider information, or remove a listing you own. Email hello@mntherapyfinder.com and we'll take care of it.
